Sean Nelson décroche à l'âge de 10 ans un rôle dans la pièce de théâtre Hey Little Walter. Il fait ses débuts à la télévision dans un épisode de la série Here and Now de NBC en 1992, avec Malcolm-Jamal Warner.
Il a fait ses débuts au cinéma dans Fresh (1994), dans le rôle du personnage principal Michael (surnommé Fresh). Sa performance dans le film a été accueillie positivement, les critiques décrivant Nelson comme " la clé du succès du film ". Il a remporté un prix au Festival du film de Sundance et a également reçu une nomination au Young Artist Award pour son travail dans Fresh.
Tout au long des années 1990, il est apparu dans des rôles invités dans les séries policières Law & Order, Homicide : Life on the Street, et New York Undercover.
